Over the past century, local anaesthetics and their delivery systems have evolved and improved significantly. Despite these advances, our ability to obtain profound numbness 100% of the time can be elusive, especially in the mandible. These situations can create painful dental visits for patients, creating anxiety for them and for the operator as well. This seminar will explore the reasons and remedies for incomplete local anaesthesia. A discussion about relevant nerve and bone anatomy and pharmacology that can lead to incomplete local anaesthesia will be presented. Techniques to supplement the standard inferior alveolar nerve block and armamentaria besides the standard needle and syringe will also be evaluated.

Some of the topics to be explored could include:

  • Local anaesthetic uffering techniques.
  • Pain reduction ideas.
  • The latest research about articaine.
  • Local anaesthetic toxicity, allergy and drug interactions.
  • Other injection approaches: intranasal; PDL; intraosseous; Gow-Gates block; and computer-controlled devices.
  • Vasoconstrictor considerations for healthy and cardiac patients.
